Retroperitoneal Lipoblastoma Involving the Right Common Iliac Artery and Vein

Abstract

A case of benign lipoblastoma of the retroperitoneum in a 12-month-old boy, presenting as a huge abdominal mass and right lower extremity swelling, was reported. Surgery revealed involvement of the right common iliac artery and vein by lipoblastoma, requiring the sacrifice of both vessels for complete removal of the tumour. Vascular continuity of both vessels was re-established by a saphenous vein graft interposition. Involvement of a great artery by benign lipoblastoma has not been reported before.

Résumé

Nous présentons un cas de lipoblastome bénin du rétropéritoine chez un garçon de 12 mois. L'examen clinique révélait une masse abdominale volumineuse et un œdème du membre inférieur droit. A l'intervention le lipoblastome envahissait l'artère et la veine iliaques communes droites nécessitant le sacrifice des deux vaisseaux pour obtenir une résection complète de la tumeur. L'interposition d'un greffon veineux saphène a permis de rétablir la continuité vasculaire. A notre connaissance, ceci est le premier rapport d'un envahissement d'une artère majeure par un lipoblastome bénin.

Resumen

Presentamos un caso de lipoblastoma benigno en el retroperitoneo de un niño de 12 meses con una masa abdominal de gran tamaño y tumefacción de la extremidad inferior derecha. Durante la operación se encontró un lipoblastoma que afectaba a la arteria y vena iliaca primitivas derechas y que requirió el sacrificio de ambos vasos para completar la resección. Se restableció la continuidad vascular con injertos de safena. No ha sido publicada hasta ahora la afectación de un gran vaso arterial por un lipoblastoma benigno.

Zusammenfassung

Es wird ein Fall eines Lipoblastoms des Retroperitoneums bei einem 12 Monate alten Knaben, der sich mit einem abdominellen Tumor und einer Schwellung im Bereich der rechten unteren Extremität vorstellte, dargestellt. Während des operativen Eingriffs zeigte sich, dass die rechte Arteria iliaca communis sowie die rechte Arteria vena iliaca communis vom Lipoblastom ummantelt waren, so dass beide Gefäße, um eine komplette Resektion zu erreichen, durchtrennt werden mussten. Die Kontinuität beider Gefäße wurde mit einem Vena-saphena-Transplantat gewährleistet.

Schlussfolgerungen

Auch bei einem Lipoblastom ist es möglich, dass die großen Arterien in den tumorösen Prozess mit einbezogen sind. Dies wurde bisher nicht berichtet.
